What exactly is market based when they are extracting as much profit as possible?

I was right.

1. Proactive, Basic & emergency care for all, free clinics, hospitals, home visits
- paid for by payroll, employer tax, flat rate, no cap.
- staffed by Drs, Nurses, etc that are paying back their free education
- but at some point, care hits a limit

2. Special fund for keeping poor older people alive longer,
- funded by a mix of charity and a special tax only on wealthy older people

3. Special fund for extreme cost cases for the rest of us
- funded by a mix of charity and Pharma profit tax (diversion of some of what they pay now)

4. Free market
- you want something better/different, you buy what you want.
- your employer wants to incentivize you with a Cadillac plan? OK
- could be taxable as income (to help fund the rest) or tax free to draw more people away from the free stuff.

Can't afford then you do without.

Insurance priced to risk, like property insurance.
